Key 2025 AHA Guidelines
- High-quality CPR is primary treatment: No shock indicated; focus entirely on compression quality
- Systematic H's and T's search: Hypoxia, Hypovolemia, Hydrogen ion (acidosis), Hypo/Hyperkalemia, Hypothermia; Tension pneumothorax, Tamponade, Toxins, Thrombosis (coronary/pulmonary)
- Point-of-care ultrasound: Valuable for identifying reversible causes (tamponade, PE, hypovolemia)
- Epinephrine timing: 1mg IV/IO every 3-5 minutes; earlier administration may improve outcomes
- Consider thrombolytics: For suspected/confirmed PE causing cardiac arrest
Bhagavad Gita Teaching
Verse 18.78: "Yatra yogeshvarah krishno yatra partho dhanur-dharah, Tatra shrir vijayo bhutir dhruva nitir matir mama" ("Wherever there is Krishna, the lord of yoga, and wherever there is Arjuna, the wielder of the bow, there will surely be fortune, victory, prosperity, and sound judgment.")
Application: In PEA/asystole—the most challenging arrests—success requires the combination of technique (Arjuna's bow) and wisdom (Krishna's guidance). We must search systematically for the reversible cause.
The Integration
PEA and asystole represent the ultimate test of clinical wisdom and persistence. Unlike VF/pVT where we can shock, here we must be detectives, methodically searching for the underlying pathology while maintaining perfect CPR. The Gita verse reminds us that victory comes from combining skill (technical excellence in compressions, airway, access) with discernment (systematically evaluating H's and T's). Krishna's "yoga" represents the 2025 emphasis on systematic approach and POCUS, while Arjuna's "bow" symbolizes our interventions—each compression, each dose of epinephrine, each corrective action. The promise of "dhruva nitir" (sound judgment) encourages us to think clearly even in these seemingly hopeless rhythms. Many PEA arrests are reversible if we find the cause; our dharma is to search systematically rather than mechanically continuing futile CPR.
